Gun-barrel straight means perfectly straight, as straight as anything could be with maximum precision. The allusion is obviously to a gun barrel, which has to be straight to be effective. The phrase is often used in a sporting context to describe the trajectory of a ball. The OED confirms that the first citation is American from 1938.
