It means that something makes no difference or does not matter and dates from the early 20th century. It is of American origin, allegedly from the sport of boxing. Various parts of the anatomy are often substituted for nose e.g. back, elbow etc but no skin off my teeth is thought to be a confusion with by the skin of one’s teeth.
