Topsy-turvy

Upside down, confused or all over the place dates from the early 1500s. Topsy is undoubtedly derived from top and turvy is probably, according to the OED, derived from the obsolete medieval verb terve meaning to overturn or topple over. See also higgledy-piggledy.
